What is RPM?

From CMS, RPM “involves the collection and analysis of patient physiologic data that are used to develop and manage a treatment plan related to a chronic and/or acute health illness or Condition.”

Allows patients to measure and securely transmit vital data to their provider to ensure they’re managing their health in accordance with their care plan. Clinicians can make timely adjustments as needed.
